Requirements:
4. Conduct person-centred assessments and develop care plans to promote independence and meet identified needs within resource limits.
5. Collaborate with individuals, carers, and other professionals, ensuring care services are person-centred and strength-based.
6. Manage a caseload of adults with complex issues, safeguarding their safety and well-being while maintaining accurate records.
7. Embed personalisation in practice, focusing on individual budgets, direct payments, and outcome-driven support plans
8. Undertake safeguarding investigations, manage risks, and contribute to service development while adhering to council policies and professional standards.
What we are looking for in a Social Worker:
9. Social Work England registration
10. Eligible to work in the UK
11. Hold a full UK licence
12. Post-qualified experience
13. Degree level or equivalent in Social Work
