Frailty Hub Social Worker
Job description At The City of York Council (CYC), The Adult Social Care Community Team (ASCCT) are working in partnership with Nimbuscare, an at scale provider of Primary Care and we are recruiting an experienced Social Worker to work as part of the York Frailty Hub. This is an innovative joint post between CYC and Nimbuscare. This post holder will be employed by CYC but based at Holgate Park Drive, as part of a multi-disciplinary team. This is an opportunity to be part of a new and dynamic programme of work. York Frailty Hub is a clinically led, multi-organisational team supporting frail patients in the community through the delivery of a Frailty Advice & Guidance (A&G) Line and the coordination of multi-agency, joined-up, community-based response.
The York Frailty Hub aims to:
Reduce avoidable hospital admissions Expedite safe discharge from hospital by means of a discharge to assess bridging service alongside a virtual frailty ward. Enable people to remain independent for longer and optimise their quality of life whilst living in the community.
This is a new post and we are looking for someone who has the ability to work in a fast paced environment as part of multi organisational team. There will be opportunity for the right candidate to grow and develop this role, so we are looking for someone who can adapt positively to change as part of a fast-growing service.
As a CYC social worker, based in the York Frailty Hub you will be a key member of the multi-disciplinary team that is provided through the York Frailty Hub for people living in the City of York. As part of the York Frailty Hub MDT you will complete assessments, assess capacity in relation to care and support needs, provide safeguarding advice and guidance, and proactively support admission avoidance. The role involves working closely with health and social care partners, which include York Integrated Care Team, Community Response Team and other universal services such as voluntary agencies in the city.

As a CYC employee you will have line management support by an ASSCT team manager. This includes support via attending team meetings, training, progression opportunities and also updates to changes with service improvement. Adult Social Care at CYC has a clear vision and we are committed to integration. Our practice framework supports high-quality practice by providing clear expectations within which a consistent approach can be applied.
To apply you will need a degree in Social Work and be registered with Social Work England (SWE). Currently we are not licenced to sponsor employees for visas, therefore are unable to offer sponsorship for these roles.
As this role is public facing, applicants will need to demonstrate, at interview, their competency to converse and provide advice and guidance to members of the public, in spoken English to CEFR level C1: Can express him/herself fluently and spontaneously, almost effortlessly. Only a conceptually difficult subject can hinder a natural, smooth flow of language.
