Job summary The role of the Bariatric Nurse Specialist within the Trust is to support patients and specialist obesity services, which include surgical interventions. The CNS team work closely with the professionals to provide our patients with pre and post-operative support, helping to manage symptoms, side effects and also offering emotional support. The opportunity has arisen for us to recruit a dedicated professional who would like to be given the opportunity to make a difference to their patients. The post holder will continue to develop the Bariatric CNS Nursing service, ensuring this encompasses the needs of the patients and is in line with Standards of good practice. The service is managed across multiple sites and the successful post holder must be able to work from all locations to meet the needs of the service. Main duties of the job Provides a specialist and holistic assessment of patients and carers needs following referral. Provides specialist care and advice for patients and carers and assists the multidisciplinary team in delivering and evaluating the interventions/ plan of care, reviewing and amending this plan as appropriate. Acts as a key-worker in co-ordinating care between different professionals and agencies. Instigate actions to the development of a relationship between the patient/ carer and members of the MDT, in order to enhance communication. Frequently initiate and participate in sensitive communication issues with patients, carers, nursing/medical teams. Attend bariatric clinics providing specialist advice and support. Visit surgical inpatient wards to liaise with surgical teams caring for patients undergoing Bariatric surgery. Participate and contributes to multidisciplinary team meetings. Develop specialised nurse led care, in line with national recommendations. Assists in the development, implementation guidelines and pathways to ensure each patient undergoing Bariatric Surgery has a comprehensive management plan. Review and develops appropriate patient documentation and information in line with Trust policy and Department of Health recommendations.
