We are currently looking for community nurses in work in our district Nursing teams based in South Westminster Centre for health.
This post offers both experienced and newly qualified nurses the opportunity to develop clinical skills, ranging from wound care, palliative care, diabetes care, IV therapy, just to mention a few.
Main duties of the job
The successful candidates will have a full induction trust and service induction. This will enable you to gain meet other staff in the Trust who are not from your service to enhance knowledge and help you develop working relationships as well. You will also receive support from the Trust induction facilitator and practice development nurse who will support you with initial and on going development.
To provide skilled nursing care working under the guidance of the District Nurse (DN) team lead as part of the Primary Health Care Team, ,to patients in a variety of community settings between the hours of 0830 and 2100 hours.
To monitor, maintain and develop the service provided.
With support, to take responsibility for managing the caseload and the team in the absence of the DN team lead. This will include access to another team lead for clinical and practice advice.

* Recent clinical nursing experience negotiating and liaising within a multi-disciplinary team
* Working with people with complex health needs
* Supporting students and other learners e.g., HCAs teaching others in the practice environment
* Experience of undertaking general assessment by utilising evidence-based care.
* Previous experience in the Nursing process, care planning and fluent in record keeping
* Experience of providing holistic carethat involves the multidisciplinary team
Desirable
* Experience of training other students.
* Participation in audit research projects service development
* Post registration experience in a NHS community environment
