Job Overview
The district nursing service is fundamental in delivering the health board’s clinical futures model, providing care closer to home, enabling early discharge, and preventing hospital admissions. We deliver holistic care to patients within their own homes and residential homes, meeting the increasing complexities in the health of our patient population. Assessment, planning, and delivering high standards of care are achieved through effective collaboration and partnership working within our health and social care integrated centre at Chepstow Community Hospital, including social services, the district nursing team, GP surgeries, and many other health and social care professionals.
Main duties of the job
To care for patients and their families within their own homes is a privilege. The Chepstow District Nursing Team has an exciting opportunity for a community staff nurse to join our 24/7 service and be part of a supportive, dedicated, and forward-thinking team. You will have the opportunity to develop your clinical and interpersonal skills with continuing professional development opportunities to prepare you for the evolving autonomous role of the community nurse through engagement in a robust induction programme.
