Job summary An exciting opportunity has arisen to work as a Band 6 District Nurse / Case Manager in SMASH Care Community Team. This is a 12 month fixed-term position covering maternity leave. Main duties of the job In the role as a District Nurse / Case Manager you will work in partnership with our patients and their families to lead in the delivery of person centred, safe and effective care. You will have extensive clinical experience and experience of case management, preferably within the community setting and be able to demonstrate recent and ongoing clinical professional development relevant to this post. Although the post is based in Sandbach, there will be at times a requirement to work across the footprint of Central Cheshire Integrated Care Partnership.
