Job summary The Mount View Practice in Fleetwood are looking for an energetic, dedicated, caring and patient focused individual with a can-do attitude, who has the desire to make a difference to the lives of the patients in our community. The HCA will be working under the direct supervision of our Practice Nurses and strictly in accordance with specific Practice guidelines and protocols. The HCA will assist the Practice Clinical Team in the provision and delivery of prescribed programmes of patient care. Ideally you will have the skills for a Primary Care environment including competency in phlebotomy and working with patients with long term condition such as diabetes. But full training will be given to the successful candidate. Must be able to work Mon to Fri between the hours of 0800 and 1830. Main duties of the job New Patient Medicals NHS Health Checks Phlebotomy Weight Management Pre diabetic checks Electrocardiogram Annual reviews of long term conditions Carry out urinalysis and preparation of specimens for investigation by the pathology laboratory Clear and restock clinical consulting rooms Ensure vaccines and cold chain storage monitoring and recording is carried out as per policies and protocols Prepare and maintain environments and equipment before, during and after patient care interventions including assisting GPs during the performance of minor operations Help to raise awareness of health and wellbeing and how it can be promoted Act as chaperone when required Home Visits Where appropriate, provide support and encouragement to Patients to self-manage their conditions The above is not an exhaustive list of expected duties to be undertaken.
