The role: Join a dynamic and supportive team and embark on a fulfilling journey as a Community Reablement Assistant. In this flexible and rewarding role, youll play a vital part in helping individuals regain their confidence and independence after a hospital stay. As the first point of contact for those returning home, youll provide essential support and bring positivity to their recovery journey, making a meaningful difference in their lives every day. Your main responsibilities: Supporting people with their personal care and empowering them to do as much as possible on their own. Monitoring and providing feedback on the progress of peoples goals. Assisting with medication and encouraging independence. Completing care records and other relevant paperwork. Facilitating access to community groups and services. No prior care experience? No problem Well provide all the training and support you need to succeed and grow in your role.
