Are you a nurse looking for a supportive team and the opportunity to develop new skills? This is an exciting time to join the Twilight Community Nursing service in West Herts. We are developing new community pathways to support the delivery of more care in people’s homes, with a range of new learning and development opportunities available through the CLCH Academy.
We are looking for a part-time nurse with a genuine interest in continuing to develop skills in this field. You must be able to work flexibly according to service needs and have excellent organisational, time management and communication skills.
You will be based at one of four sites within the Herts Valley area: Watford (BRE), Hertsmere (Potters Bar Community Hospital), St Albans and Harpenden (Harpenden Memorial Hospital), and Hemel Hempstead (The Forum, Marlowes).
Main duties of the job
The Community Nurse will provide a mixture of general and specific care to a group of patients under the guidance and supervision of a registered nurse or allied health professional. The Community Nurse will work in patients' homes as well as visiting patients in residential homes.
The post holder will be expected to become proficient and competent in their area of practice by undertaking competency assessments specific to the area of clinical practice. The Community Nurse will promote optimum independence and individualised client-centred care to a delegated caseload.
The post holder will also support the Community Nursing Team to deliver against key performance indicators and maintain quality of the service. The service is provided seven days per week from 4pm to 8pm covering the four localities across Herts Valleys.

Education/Qualification
* Willingness to undertake training as appropriate.
* Evidence of other post-registration education and training.
Skills and Knowledge
* Knowledge of NHS
* Demonstrate an acceptable level of numeracy and literacy.

* Knowledge of the effects of the ageing process and needs of vulnerable adults in relation to their health and well-being.
