Job summary Trinity Ward is a 25-bed community rehabilitation ward. We promote patient involvement and empower patients and family members to provide input into their clinical care and decisions. As well as rehabilitation Trinity Ward also provides palliative care beds for those reaching the end of life. We provide dignified holistic care for the patient and their families in a calm and caring environment. Main duties of the job The role of the assistant practitioner is to assist patients with their hygiene needs, toileting, nutritional intake, supporting and encouraging patients with their mobility and monitoring and recording vital signs, whilst maintaining their dignity and respecting their individuality. To actively contribute to the effectiveness of team working in the nursing and rehabilitation of patients within the service and participate in MDT meetings and case reviews as required. To provide profession specific nursing or rehabilitation care to patients/clients. To carry out treatment plans, under the guidance of the relevant clinical specialist in nursing, rehabilitation or therapy. To perform a range of profession specific nursing, holistic, rehabilitative or therapy interventions. To assess, observe, monitor, document and report any changes in the patients' condition to senior member of the team, using own judgement and initiative. To enable clients to maintain and improve their independence and use rehabilitation/therapy aids To provide advice, support and have the ability to motivate, encourage and persuade patients to participate in their programme of care and/or rehabilitation/therapy To provide support to the family carers and to provide comfort to relatives of palliative patients.
