Job summary We are looking for a dynamic, self-motivated individual to join our clinical pharmacy team as our Advanced Pharmacist - lead for anticoagulation service and thrombosis. You will be responsible for leadership of the anticoagulation service with direct responsibility for management of the anticoagulation team ( pharmacists, nurses and healthcare assistants) in addition to being the Trust lead pharmacist for thrombosis. The post holder will provide inreach and outreach services for patients ensuring continuity of care during hospital admissions. Ideally having experience of anticoagulation and thrombosis treatments, management of staff and also service delivery. In addition to excellent communication and leadership skills, you will have previous experience of working within a clinical environment. You will need to be confident collaborating with clinicians, managers, nurses and others to ensure that pharmaceutical care of patients is optimised. You will have the opportunity to work with both GPs and commissioners in ensuring a safe anti-coagulation service is delivered. Main duties of the job You will be registered as a pharmacist with the General Pharmaceutical Council and have successfully completed a Postgraduate Diploma in Clinical Pharmacy Practice or be able to demonstrate equivalent and transferrable experience. You will also be a registered non-medical prescriber. Training and support will be given and a commitment to continuing professional development is essential. The role will include participation in weekend working and may include on-call commitment. Applicants should have knowledge of therapeutics and medicine use and experience of audit, risk management and the ability to communicate effectively with a wide range of staff. This role is critical to providing a high quality clinical service to patients.
