The Patient Flow Coordinator is a key part of the Integrated Discharge Team (IDT) working in a patient facing, ward based role.
They will work under the direction of the senior members of the IDT and in conjunction with the ward multi-disciplinary teams (MDT).
They will assist with planning and implementing patient discharge; as well as aiding general flow of patients through the hospital., An exciting opportunity has arisen for Patient Flow Coordinator in the integrated discharge team working across both sites of Mount Vernon and Hillingdon Hospital.
This role will require a dynamic approach to discharge planning and processes that will ultimately improve the length of stay to the patient and will support safe and time discharges.

You will be a compassionate, enthusiastic and confident individual, who is able to provide (following training) the highest standard of patient care, under supervision. The Hillingdon Hospitals NHS Foundation Trust is the only acute Hospital in the London Borough of Hillingdon and offers a wide range of services including accident and emergency, inpatient care, day surgery, outpatient clinics and maternity services.
