Health Care Assistant - HCA - a variety of hours are available. Previous Health Care Assistant experience is desirable. 6 weeks annual leave.
York Medical Group is a large, inner city practice, serving a population of 44,000 over 6 sites. Our diversified workforce includes GPs, ANPs, UCPs, Nurses, Assistant Practitioners, Health Care Assistants, Care Coordinators, Link Workers, and Mental Health Workers.
Main Duties and Responsibilities:
1. Provide Health Care Assistant services to the practice population, supported by the nursing teams.
2. Deliver care within the boundaries of role and in accordance with specific practice guidelines and protocols.
3. Demonstrate critical thinking and skills in clinical decision-making within the parameters of qualifications and role requirements.
4. Support patients to be healthy by assessing their needs, providing HCA care, health education advice, and screening activities.
5. Implement care at the surgery and in community settings when required, running a variety of clinics.
6. Maintain clinically accurate records.
7. Work collaboratively with the general practice team to meet the needs of patients and support the delivery of policy, protocols, and procedures.
8. Make professional decisions, providing safe, evidence-based, cost-effective individualized patient care, including health checks, blood pressure, temperature, height, weight, BMI, and blood sugar levels.
9. Take and prepare pathology samples, including venepuncture, INR, and urinalysis.
10. Undertake ECGs and assess, plan, develop, implement, and evaluate programs to promote health and well-being.
11. Support patients to adopt health promotion strategies and encourage self-care principles.
12. Provide holistic service to patients and their families, developing ongoing care/support plans with an emphasis on prevention and self-care.
13. Provide education and advice related to basic ear care and undertake ear syringing when clinically indicated.
14. Administer B12, Influenza, Pneumonia, and shingles injections under the provision of Patient Specific Directives.
15. Manage chronic diseases by undertaking appropriate investigations and clinical observations.
16. Meet the needs of patients presenting for wound care and provide basic wound care and removal of stitches and clips.
17. Prioritize health problems and intervene appropriately in complex, urgent, or emergency situations.
18. Recognize patients who may be at risk and raise concerns appropriately.
19. Ensure adherence to infection control policies and procedures.
20. Maintain adequate stock levels in all clinical rooms and ensure daily, weekly, and monthly checks are completed.
21. Participate in audits where appropriate.
The successful candidate may be required to work across sites on occasion, requiring flexibility. Some extended hours and Saturdays will be expected on a rota basis.
