We are looking to recruit Dispensary Pharmacist roles based within our purpose-built pharmacy department at Maghull Health Park and our new in house Pharmacy at Hollins Park Hospital in Warrington. Working hours, base and location of work would be mutually agreed following on appointment in role. Shortlisting planned for: 9 September 2024 Interview planned for: 19 September 2024 Main duties of the job - To work alongside the dispensary manager to manage the daily workflow within the dispensary, ensuring that patients receive safe and appropriate medication in a timely manner. - To manage and co-ordinate the clinical screening of prescriptions within the dispensary, ensuring medications prescribed are safe, appropriate and cost effective. This includes delegating workload to other pharmacists. These prescriptions include but are not limited to: o Inpatient orders o Discharge prescriptions (TTOs) o Planned work for depot or clozapine clinics o Outpatient prescriptions To act as the pharmacist lead with internal departments and teams including pharmacy procurement, digital, clinical and quality teams. To deal with professional queries within the dispensary and provide advice to healthcare professionals, carers and service users. Recruitment and retention premia apply to these posts. Successful candidates will receive a £2,000 payment on completion of induction and an additional £2,000 on completion of the first year of employment.In addition, there is a retention premium payment of £2500/annum for the duration of the retention premium being available. Job responsibilities Our dispensary-focussed clinical pharmacist roles provide a great stepping stone for experienced community pharmacy dispensary-based Pharmacists considering transitioning into a hospital-based role. 1. To be responsible for implementing the Medicines Management strategy in the Trust.2. To deputise for a higher grade of pharmacist as required3. Participation in rota at specialist level which will include Medicines Information and providing professional and legal supervision to the dispensary services.4. To participate in the Trust-wide Out of Hours Medicines Advice Service and 7 day supply service.5. Completing with the support of technicians the process of medicines reconciliation following admission or transfer of service users within the trust. This involves collating information from patients and carers and liaising with professionals from other healthcare organisations to ascertain presenting complaint, past medical and medication history, working diagnosis and relevant biomedical and psychiatric test results to ensure the safe and effective use of medicines.6. Ensuring that each medicine has an appropriate indication and each diagnosis has appropriate therapy.7. Ensuring that existing therapy is safe to take with, and has not contributed to, the presenting complaint.8. Ensuring that new treatments are compatible with the patients conditions.9. Making recommendations on changes to drug therapy in line with the above.10. Reviewing recommendations in the light of new results and treatment plans.
