About the Role
Mid Norfolk Primary Care Network (PCN) has an opportunity for a care co-ordinator to join their team, supporting six GP practices and linked care homes across the Dereham area, this position can be considered on a full and part time basis.
The role involves working very closely with the GPs and practice teams within the PCN, and is pivotal in ensuring that all patients receive the best possible care and service.
The main duties of the role include:
1. Act as a central point of contact to ensure appropriate support is available.
2. Plan and organise work using own initiative, whilst being able to work as a valuable team member.
3. Be able to make referrals to relevant service providers.
4. Coordinate meetings, having proactively prepared any actions prior to the meeting and ensure all relevant clinical and non-clinical staff are present.
5. Promote and facilitate integration within the GP and community teams.
6. Provide and receive sensitive information about difficult or complex matters always respecting confidentiality.
7. Create detailed and clear minutes from the meetings and forward to all relevant parties with actions highlighted to individuals.
8. Where required, be able to offer appropriate support and guidance to patients and families/carers.
9. Receive, record, and collate information and maintain accurate electronic records of patient care and planned service provision, including hospital admission and discharge information.
10. Signpost service users and carers to relevant services.
11. Contribute to quality audits.

If you are a skilled PCN care co-ordinator, with excellent person-centred planning experience and strong local knowledge for accessing key support, then we would love to hear from you.
The ideal applicant will have the following skills, experience and qualifications:
Essential:
12. Excellent working knowledge of Microsoft Office Packages.
13. Experience of dealing with sensitive/confidential information.
14. Proven administrative experience.
15. Understanding of Clinical Governance.
Desirable:
16. Experience of working in a healthcare setting.
17. Experience of office procedures within an administration role.
18. Working knowledge of the management of databases.
19. Experience of working within Multidisciplinary teams.
20. Knowledge and understanding of relevant health and social care legislation and initiatives.
