Coventry Community Palliative Care Team supports patients, families and carers with specialist palliative care needs, palliative support, and end of life care within the community setting. The post holder will be required to support the clinical lead to our already existing Rapid Response team, using their excellent specialist knowledge and skills and being a role model by providing support, education, and excellent clinical leadership. The post holder will work as part of the integrated palliative care multidisciplinary team and will work with other health care professionals to provide specialist advice, support, and high-quality patient centred care to those affected by malignant and non-malignant disease at the end of life within a 2-hour rapid response. This role is a clinical role as part of a 7 days a week 0800 - 1600 on a shift rota basis provision.
Please note that a full UK valid driving license and access to a vehicle that can be used for work purposes are essential for this role.
Main duties of the job
* To demonstrate and provide excellent clinical leadership skills, being a professional senior role model to the Rapid Response team.
* To provide ongoing support, specialist education, supervision and guidance to the Rapid Response team and where appropriate other health care professionals.
* To provide direct clinical urgent assessment and advice for patients with cancer or an advanced terminal illness and their families, who require specialist palliative care.
* To develop strong links with the local GPs, District Nurses, local Hospices, and other allied health professionals.
* To act as a specialist resource for community and primary care colleagues in the management of patients with specialist palliative care needs.
* To undertake appropriate service development (QI projects etc) and review of policies, procedures and protocols within the Trust related to palliative care.
* Actively manage unscheduled palliative care to patients in a 2 hour & 24 rapid response timely manner.
* To safely refer patients on to the relevant health care provider as and when appropriate.
* To manage the palliative care phone out of hours/weekends.
* To escalate any concerns or support needed to the service manager and or appropriate MDT member.
* To undertake continuous evidence based learning required for the position.
* To support the clinical lead of the team.

Job responsibilities
* Develops and maintains highly effective working relationships with relevant clinical nurse specialists, social care providers, local hospices, GPs and other community health professionals in order to promote and sustain a multidisciplinary/multi agency approach to planned, co-ordinated care.
* Be able to use a variety of strategies and a range of advanced communication skills to relate effectively with patients (when breaking bad news, dealing with anxious/bereaved relatives) and those involved in their care.
* To identify the need for and initiate the use of translator services as required.
* Work as an autonomous practitioner within a multi-professional team, supporting patients at end of life with complex and specialist palliative care needs, recognising own limitations and seeking support from senior members of the specialist palliative care team.
* Work in partnership with GPs and consultants to ensure the provision of seamless specialist palliative care.
* To provide a 7-day CNS service to the integrated palliative care team.
* To provide a 2-hour & 24 hour rapid response to palliative and end of life patients.
* To demonstrate professionalism, excellent leadership skills and role model skills at all times.
* Prioritise own workload on a day to day basis and that of your team by taking guidance and support from the daily coordinator.
* To take part in coordinating and delegating the palliative care phone.
* To manage and delegate unscheduled care.
* To contribute to nurse led assessment of new and follow up patients where appropriate to do so.
* Makes sure that people affected by cancer and any palliative life limiting condition are aware that they are interacting with a UHCW Palliative care Professional and know about the full range of resources and services available.
