Job summary
To work as part of the Acute Older Peoples MentalHealth Service, focusing on the direct needs of service users. Delivering individual treatment plans acrossboundaries of care, working closely with Care Co-ordinators through the careprogramme approach and care management process.
Main duties of the job
To providesupport, give time to service users within the unit and the community and thuspromote recovery. To provide holisticcare, taking into account psychosocial and physical healthcare needs of theservice user group.
To assistCare Co-ordinator or key worker and service user to assess, plan, implement andevaluate individual care plans and undertake other assessments as required.

Job description
Job responsibilities
The PostHolder must have the individual service users needs at the fore at all timesand use the agreed set of values and skills to underpin their day to day work.
Supervise andsupport junior staff (Band 2) in their roles, providing advice and acting as arole model and mentor.
To workwithout direct supervision, but with someone available to refer to, as and whenrequired, either on or off site on a regular basis, and to take charge of anarea in the absence of qualified staff, making judgements around service usersconditions, at that time, and the requirements of the area.
Ensure thatservice users engage effectively with the agreed care plan and acrossappropriate services provided on a regular and consistent basis.
Will plan andsupervise individual or group therapy sessions, in line with the agreed careplan or programme of care.
Provideregular and practical support to service users and their carers/significantother in developing and managing dignity and independence.
Providesupport and assistance, as necessary, with daily living activities includingpersonal care.
The PostHolder will be involved with facilitating community and hospital appointmentswhen necessary.
Positivelypromote independent living of service users within the community.
Work flexiblywith Key Worker, Care Co-ordinator, other relevant community support, serviceuser and significant other to implement plan of care.
Will behandling service users cash and valuables on a regular basis.
Help identifyearly signs of relapse by monitoring the services user progress, level offunctioning and mental state and alert the appropriate staff involved in thepersons care.
Assist theCare Co-ordinator/Key Worker and service user in developing crisis/contingencyplans and advanced statements, as per local policy guidelines.
To maintainadequate records, as required, by existing procedures, entering appropriatedetails on the service users case notes, as necessary.
To be able tocommunicate routine information in an arena where there will be barriers tounderstanding, and to be able to use tact and persuasion in a non aggressivemanner.
Provideinformation on health promotion and wellbeing.
To ensure theservice user understands and has a clear pathway of care across sector/agencyboundaries with key contact points/named individual.
Attend andparticipate in agreed training, development and supervision sessions as anindividual and as part of the team.
To undertakesuch other duties as may be determined from time to time within the generalscope of the post.
Adhere tohealth and safety legislation.
ThePost Holder will be required to have access to a vehicle for business purposes.

Qualifications
Essential
1. NVQ Level III or Certificate in Community Mental Health, or a willingness to undertake this, or an equivalent level of experience.

2. A basic knowledge/understanding of the Mental Health Act and Mental Capacity Act
