Duties specific to this role:
1. Work as an integral part of the Children, Young People and Families Directorate to deliver services directly to members of our community.
2. Ability to deliver services within the community.
3. Provide support to people within the boundaries of our services, identify any additional support, and sensitively help them to access other services as needed.
4. Be responsible for managing a caseload of young people accessing the service and overseeing their care planning.
5. Work alongside other staff and volunteers to ensure members of our community can access the services they need.
6. Make decisions autonomously according to good clinical practice to ensure the safety of people accessing services and help identify areas for improvement.
7. Maintain professional boundaries and work with partner organisations to safeguard people, ensuring safeguarding practices are followed and embedded within personal practice.
8. Work in collaboration with the Primary Care Network in Oldham West and other agencies across Health and Social Care to develop bespoke care plans.
9. Represent the organisation and our values during your work with CYP and families, at local events and celebrations as well as at relevant meetings such as CIN, Child protection, team around the family.
General duties:
1. Communicate effectively, verbally and in writing, to a wide range of people inside and outside the organisation.
2. Use IT systems including email, online systems, and Microsoft packages to record and share information.
3. Deliver and monitor a range of complex activities or programmes in your department and achieve service and project outcomes.
4. Support the organisation to devise and develop improvements in your area of expertise.
5. Work in partnership with a wide range of partners, stakeholders, and communities.
6. Engage in continuous professional development and carry out any other reasonable tasks relevant to the role.
