Midland Partnership University NHS Foundation Trust provides a comprehensive range of mental health and related services to the population of Staffordshire and Shropshire.
Ellesmere House is a 12 bedded low secure Learning Disability unit. We cater for individuals who have complex needs and include primarily a learning disability but may also include autism and/or mental health issues. The client group may also but not exclusively require partnership working with agencies such as the Criminal Justice System and Home office.
Ellesmere has a full range of specialized provision including our own consultant, psychology, social worker and an enhanced OT/activities coordinator provision. We are an integrated team and very much work together as a team. The nursing team is a committed group as are the entire team overall as a unit.
The Hatherton Centre is a 47-bedded medium-secure facility incorporating an intensive care ward, acute assessment ward and rehabilitation wards. The Clee unit (in Shropshire) is a 32 bedded low secure mental illness unit which includes a rehabilitation service and an acute assessment service and a multi-agency community service, liaison scheme, out-patient facilities and court assessment and engagement schemes.
We would welcome applications from both newly qualified and more experienced nurses qualified in Learning Disability Nursing (RNLD).
Main Duties and Responsibilities
1. To act as key worker or CCR to an individual or group of service users, coordinating all aspects of care in accordance with the Care Programme Approach (CPA).
2. Assessment of patients' individual care needs.
3. The development of programmes of care.
4. Implementation of agreed programmes of care.
5. Evaluates effectiveness of programmes of care.
6. Ensures compliance with NMC Code of Conduct.
7. Measures quality of care by the use of appropriate tools.
8. Implements Mental Health Act (1983) appropriately.
9. Implements local and national policies.
10. Promotes and maintains effective relationships with other disciplines.
11. Promotes and maintains effective relationships within the ward team.
12. Takes charge regularly of shift in ward environment.
13. Acts as primary nurse for Clinical Team embracing multi-disciplinary working.
14. Identifies training requirements of self and junior staff.
15. Maintains an up to date level of professional competence.
16. Assess, develops, implements and evaluates ward teaching programmes, in conjunction with responsible nurse.
17. Supervises and organises student nurses and junior staff.
18. Maintains effective multi-disciplinary/agency working relationship.
19. Takes all possible precautions to safeguard the welfare and safety of staff, patients and visitors and the public, by implementing all policies and procedures relating to security and Health and Safety.
