Purpose
Shaw Trust's successful social prescribing service Connect for Health is now expanding to provide support in Ipswich Hospital to people living with respiratory conditions.
The requirement is that this role works as part of the Multi-Disciplinary Team in Ipswich Hospital and will work closely with another Connect for Health team member as well as the Making Every Contact Count (MECC) team and REACT (Urgent Community Response – Admission Avoidance Community Team) at the hospital.
The successful candidate will become integral parts of these teams, enabling a personalised care approach, focusing on the wider determinants of health and social aspects of individuals’ lives that contribute to good health and wellbeing.
To reduce the wider determinants of health, the Community Connector based in Ipswich Hospital will work closely with the Connect for Health community-based programme, to link people back into their own local community for help to access support such for financial issues, addressing social isolation or loneliness and low mood.
Essential:
You will have:
1. Minimum of SVQ/NVQ level 3or equivalent
2. Experience of working with primary or secondary care services
3. Ability to facilitate, engage, motivate and support participants
4. Able to demonstrate being highly organised, have good planning skills and be able to deliver to agreed targets
5. Knowledge of health and social care system and the application of wellbeing and prevention
