Salary: £24,432 pro rata per annum
Hours: Part time– 22.5 hours per week

PURPOSE
To support in the management of Crossroads Care services by working collaboratively with statutory and voluntary organisations to deliver support for carers and their families, in accordance with the organisation's aims, objectives, values, policies, and procedures to maintain its high standards of care.
MAIN DUTIES
To assist the Care Manager and the Registered/Operational Manager in effectively managing care staff, including:
Participating in the recruitment, induction and training of Carer Support Workers
To supervise and support a team of Carer Support Workers ensuring that their performance aligns with our organisational goals.
Contribute to the management of service delivery by:
Undertaking needs assessments and regular reviews
Completing risk assessments with ongoing monitoring and updates
Developing, implementing, and overseeing care plans
Maintaining accurate and comprehensive recording systems
Managing referrals in line with CTCWW policies and procedures
Attending internal and external meetings as required and appropriate
Reporting accidents or incidents to the Care Manager/Operational Manager
Providing signposting and completing referrals to other agencies when necessary
Ensuring the delivery of high-quality care to clients and their families.
Updating and maintaining the organisational rota database with accurate details and necessary changes.
Monitoring and addressing the day-to-day demands of the service.
Collaborating with the Care Manager and Operational Manager to develop and improve the service.
Participating in the organisation’s out-of-hours on-call system, completing required tasks during on-call shifts, and recording all actions in the monitoring system used.
