We are offering an exciting opportunity for a Band 6 Community Mental Health Practitioner (NMC Registered Nurse, HCPC Registered Occupational Therapist or Registered Social Worker) to join our dynamic and friendly Integrated Mental Health Team based in Cannock.
The Integrated Mental Health Team (IMHT) is an innovative secondary care community team that provides evidence-based psychological and medical interventions to patients with a wide range of complex mental health needs.
We are looking for a dedicated and highly skilled Mental Health Nurse/OT/Social Worker who has a keen interest in working with this client group to provide a holistic overview and lead on the care and treatment of the person's journey through to recovery.
Usual hours of work are Monday-Friday 9am-5pm. In the interest of supporting a work/life balance, we may be able to offer some working from home time within the parameters of service requirements to complete administrative duties. This can be discussed at interview.
Due to the nature of this role, you will need a full driving licence and access to a car.
Main duties of the job
You will be responsible for the management of a complex caseload, which includes specialist assessment, care planning in conjunction with service user and carer, and evaluation of individual treatment plans.
As part of this role you will also be expected to:
* Participate in the setting of quality standards, including the auditing, monitoring, and reviewing of practice in line with current clinical guidance and policy.
* Administer medication, complying with relevant Trust procedures and NMC Guidance.
* Assess patients' individual care needs, develop, implement, and evaluate programmes of care.
* Deliver a range of interventions in accordance with the agreed plan of care and Pathway guidance.
* Complete and contribute to robust risk assessments and risk management plans.
* Provide leadership with the MDT, support junior colleagues in their development, and facilitate student nurses in their training.

Person Specification
Qualifications and Training
* A recorded/registered qualification in RMN, Social Work, or Occupational Therapy.
* Active on NMC/HCPC Register.
Experience
* Experience of working with a client group who present with emotional dysregulation, high-risk challenging and chaotic behaviour, and highly complex needs relating to trauma.
* Experience of working with people who have difficulty in engaging with services.
* Experience of working in a community setting and making decisions autonomously.
