An exciting opportunity has arisen for an enthusiastic and motivated Community Support Worker to join the Integrated Community Nursing Care Team.
The post holders will provide and maintain high quality, effective, person-centred health care to people in their own homes, liaising closely with the District Nurse Team Leader and their Deputy.
Part time hours are available.
Responsibilities include:
1. Assisting in providing and maintaining high quality effective health care as part of an Integrated Nursing Team.
2. Implementing packages of care and re-enablement programmes for patients as required to maintain them in their home environment, working under guidance and supervision.
3. Providing clinical support to health care support workers.
4. Supporting case management of complex patients within the wider team.
5. Collaborating with key partners in Multi Disciplinary Team working.
6. Ensuring service delivery is in line with the Core Offer and the Standard Operating Procedures of the Community Nursing Service.
