Are you passionate about making a difference in the lives of children, young people, and families? Service Care Solutions are seeking a dedicated Outreach Family Support Worker to provide high-quality, home-based early help support using a 'whole family' approach. You will play a crucial role in preventing the need for statutory services while ensuring families have access to universal support.
* £17.00 LTD per hour inclusive of holiday pay
* 35 hours per week
* 2 month initial contract with possibility of extension after this
Responsibilities
* Hold and manage complex cases stepping down from children's social care (Level 2/3), completing early help assessments and coordinating Team Around the Family (TAF) meetings .
* Provide intensive support to children, young people, and families, including adolescents with complex emotional and mental health challenges.
* Assess risk and need using early help assessments, taking on the Lead Professional role where required.
* Develop and coordinate family support plans to reduce risk and improve outcomes.
* Support families in developing parenting strategies, promoting positive relationships.
* Maintain accurate records using the electronic case management system .
* Conduct home visits and provide outreach support.
* Deliver supervised contact sessions for families under Early Help intervention.
* Promote safeguarding in accordance with statutory guidance and legislation.
* Collaborate with multi-agency teams, including schools, social workers, and healthcare professionals.
Requirements
Essential:
* NVQ Level 3 in Childcare, Education, or Health & Social Care.
* Experience of direct work with children, young people, and families.
* Strong skills in case management, assessment, and delivering interventions.
* Effective communication skills, both written and verbal.
* Experience in providing home-based and outreach support.
* Understanding of safeguarding and child protection procedures.
* Ability to work independently and within a team.
Desirable:
* Experience in parenting support and family mediation.
* Knowledge of child development and emotional well-being .
* Experience of working with families under SEND guidelines (ages 19-25) .
* Strong IT skills and the ability to maintain accurate case records .
