Main duties of the job
An opportunity has arisen for a Band 6 Cardiothoracic Pre-Assessment Case Manager within the Cardiothoracic Directorate at Cardiff and Vale UHB which is a tertiary Cardiothoracic Centre.
This is an excellent opportunity for a Registered nurse who has extensive experience in cardiac and thoracic nursing, good communication and interpersonal skills. You will work as part of the pre-assessment team managing a case load of cardiac and thoracic patients with a dedicated team of Consultants and nurses who are driven to support and expand the service to benefit cardiothoracic patients.
The Cardiothoracic Surgery Pre-assessment clinic and Case Manager Team are based at UHW. The post holder will provide pre-operative support to patients awaiting cardiac and thoracic surgery and will be involved with clinical assessment, education and the booking and scheduling of patients. You will also work closely with the Cardiac and Thoracic surgeons in the management of patients on the waiting lists and requesting investigations, and have the ability to work alongside members of the team to provide a proactive and responsive service.
The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.

Qualifications
* Degree in Nursing or equivalent
* Evidence of current post registration development
* Health Promotion/counselling training
Experience
* Sufficient post basic registration with extensive knowledge in Cardiac Nursing
* Experience managing own caseload
* Experience in teaching and providing professional support
* Health Promotion education or rehab experience
Knowledge and Skills
* Clinical competence in cardiac nursing
* In-depth knowledge and understanding of the specialty
* Clinical leadership and people management skills
* Teaching and presenting skills
* Ability to work independently
