Main duties of the job
The role will require:
* The ability to communicate effectively with others at all levels.

* Support with the education of students from both nursing and therapy professions.
* Responsibility for the initial assessment and review of patients, across a broad spectrum of clinical conditions, considering the holistic needs of patients.
* Planning, implementing and evaluation of a person-centred care plan.
* Provide skilled clinical care, treatment, advice, and interventions, including appropriate transcribing/prescribing of medication.
* Undertake continuing health care fast track applications and checklists of ongoing nursing need and where eligible commission packages of care from external agencies.
* Be involved in triage, allocation of work and leadership of other staff.

Job responsibilities
The post holder will:
* Take responsibility for themselves and the quality of their work and the service they provide.
* Demonstrate expert clinical knowledge, health promotion and holistic assessment skills and will work with the patient and their carers to assess, plan, implement and evaluate their ongoing care and support needs in their own home or other community setting.
* Actively review, prioritise and manage the clinical caseload including triaging of new referrals over a seven-day working period.
* Participate in and undertake the annual staff appraisal and development reviews for more junior and support staff.
* Actively contribute to the task of monitoring and evaluating standards of patient care, seeking to continuously improve the quality of care.
* Work in partnership with other key stakeholders including the Primary Care Network, local authority, care agencies and the charitable and voluntary sector.
Person Specification
Qualifications
* Registered Nurse (RN1)
* Specialist practitioner District Nursing (degree / diploma) or equivalent level nursing qualification
* Non-medical prescriber (V150) or will be required to undertake this qualification on commencement in post
* Evidence of ongoing continuing professional development
* I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
* Prescribing Qualification or willingness to undertake
* Post-graduate modules in Leadership
Knowledge
* High level of clinical knowledge and skill
* Good knowledge of community services and care pathways
* Health Improvement
* Understanding of National and local health and social care strategies
* Working knowledge of the Mental Capacity Act and Care Act
* Understanding of an Integrated Care System and the Neighbourhood Teams role within the wider system.
