An exciting opportunity has arisen for an enthusiastic and committed Band 6 Memory Nurse to join our Countywide EMDASS Team. We are looking for a friendly, enthusiastic and self-motivated individual to join our EMDASS team to deliver service for North of Hertfordshire. We are a multi-disciplinary team (MDT) including Team Secretaries, Dementia Nurse Specialists, Deputy Community Manager, Service Manager, Psychologists, Psychology Assistant, Occupational Therapist, Occupational Therapy Assistants, Psychiatrists, GP, and Staff members from partner agencies. Excellent organisational and communication skills alongside solid clinical experience are essential for this post. The post holder will be expected to undertake a wide range of assessments using appropriate tools, work with the multi-disciplinary team to deliver dementia diagnosis, manage their own workload, and provide post-diagnostic support through titration reviews for service users.
Evidence of working under your own initiative, ability to problem solve, accuracy of work, and strong clinical leadership skills to support students and junior staff in creating an effective learning environment are essential. The post holder will support the transformation of the EMDASS service to improve efficiency.
Main duties of the job
The post-holder will undertake memory assessments for service users suspected of dementia, deliver diagnosis in collaboration with the multi-disciplinary team, and provide treatment and post-diagnostic support through titration reviews. The post-holder will work between One-Stop Pathway and Primary Care.
The post-holder will carry out risk assessments and implement risk management plans with service users and carers, ensuring adherence to safeguarding policies and protocols to maintain safety in the community. The post-holder will work closely with partner agencies and make tailored referrals to community services.
To act as Duty Nurse for completing the screening of referrals, offering advice and signposting.
The post-holder will undertake supervision of junior staff and trainees, as delegated by the MATS Team Leader.

Job responsibilities
The post-holder will carry out memory assessments using appropriate cognitive tools and work with the multi-disciplinary team to deliver memory diagnosis, treatment, and development of specialist care in accordance with NICE Guidelines and evidence-based practice. This role requires intense concentration.
The post-holder will work under the EMDASS One-Stop Pathway or Primary Care Pathway, or rotate between both pathways to deliver diagnosis and post-diagnosis support.
Devise care plans that promote patients' independent living in the community. Complete clear and concise risk assessments detailing management plans for identified risks.
Utilize a variety of clinical skills appropriate to individual needs and clinical settings, including home settings.
Provide ongoing post-diagnostic support for individuals as indicated by their needs.
Discharge with appropriate care provisions and follow-up, booking future review dates as necessary.
Demonstrate high standards of record keeping and documentation in adherence to Trust policies.
Collaborate with colleagues to develop and promote the service's philosophy, framework of care delivery, and strategic objectives.
Contribute to Mental Health/Dementia strategy discussions when requested by senior management.
Work with colleagues to develop clinical expertise and high standards of mental health practice.
Deliver person-centered care reflecting current best practices and challenge detrimental practices affecting older individuals.
Demonstrate understanding of legal and ethical issues in dementia care, particularly knowledge of the National Dementia Strategy.
Complete Revalidation as defined by NMC.
The post holder is responsible for management, supervision, training, and development of students/trainees and yearly appraisals.
Plan and organize diaries, prioritize duties, and coordinate care for allocated service users while considering health and safety.
Promote and support staff's personal health and wellbeing, including physical and mental health.
Access help in managing health and wellbeing.
Report any fraudulent activity and complete own extra duty and expenses claims. Ensure all applicable EMDASS policies are implemented.
Participate in service transformation and contribute to proposed changes to pathways, practices, and procedures to improve quality and efficiency.
Propose and complete audits relevant to clinical practice improvement.
Maintain high competency in conveying clinical information verbally and in writing to clients, families, and colleagues.
Demonstrate interpersonal skills, self-awareness, and empathy in communications.
Ensure adherence to patient confidentiality and documentation standards according to Trust and national guidelines.

Possess extensive knowledge of the Mental Health Act, Mental Capacity Act, National Dementia Strategy, and Safeguarding Adults procedures.

Prioritize and organize case workloads daily, ensuring high-quality care at all times.
Request colleague assistance when necessary and represent the EMDASS at meetings as delegated.
Drive to off-site meetings/training and occasionally cover for colleagues at other sites.
Spend increased time driving to clients' homes for assessments/interventions, possibly including out-of-county locations.
Participate in RESPECT and Basic Life Support Trainings as per HPFT policies/procedures.

Person Specification
Qualifications
* Degree, diploma, or equivalent in Nursing (Mental Health Nursing).
* Registration with Nursing and Midwifery Council.
* Two years post-registration experience, with at least one year in Older People's Mental Health Care.
* Post-registration course in Dementia.
* Commitment to become a non-medical prescriber.
* Understanding of end-of-life care in dementia.
* Pre or Post-registration training/short courses in mental health and/or older people's care.
* Community and inpatient nursing experience.
Experience
* Knowledge of legislation related to mental health and social care (e.g., Mental Health Act, Mental Capacity Act, National Service Framework, National Dementia Strategy, Care Programme Approach).
* Good knowledge of Microsoft Word, PowerPoint, and Excel.
* Ability to manage distressing and challenging situations.
* Expert clinical skills in dementia care.
* Experience in research and audit.
Disclosure and Barring Service Check
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975, necessitating a Disclosure submission to the Disclosure and Barring Service (formerly CRB) to check for previous criminal convictions.
Salary: £39,205 to £47,084 per annum, pro rata.
