Job Title: Team Leader
Responsible to: Libertatem Healthcare Coordinator/ Clinical Lead Nurse
The role of the Team Leader is to provide high quality care and support, as defined in the care and support plan, to our clients within the clients’ home, therapeutic environment or in the community. The successful candidate needs to be based in Chippenham. To also manage support team to ensure day to day care and strategic outcomes are met. 35 hour shift to contain 1 x 24hour shift included as a minimum.
Team Leaders are expected to:
1. Provide leadership and support for care staff.
2. Provide a weekly update via email to Clinical lead & coordinator.
3. Act as a role model for good clinical and care governance practice.
4. Uphold the rights, choice and dignity of Service Users.
5. Ensure Libertatem Healthcare Group policies and procedures are followed at all times.
6. Ensure Care Quality Commission standards are met for all clients.
Team management/ Leadership:
* Provide leadership to the team acting as a positive role model at all times.
* Motivate, support and mentor the staff when needed.
* Rota management/ annual leave.
* Monthly supervision, feedback and ongoing training needs.
* Team meetings/ minutes and dissemination of information and follow up directed steps.
* Able to have a proactive, positive approach.
Behavior and strategy management:
* Updating of care plan, risk assessments and behavior strategy plan with incorporation of professionals information.
* Monthly meetings with parents.
* Join MDTs, feedback these minutes to the team.
* Ensure meds stock been ordered.
* Bi-weekly discussion with AS to check in and give her the chance to raise concerns.
* Promoting a full and active life full of opportunities and choice.
* Someone who will have the difficult discussions with the team.
* Outcome lead goal planning in line with activity schedule.
* Monthly based Audit of Community based activities.
* All instructions given by therapists are embedded in AS life and updated accordingly.
Meds – AS online access:
* Audit of behavior management used – restraints, de-escalation techniques etc.
Supporting Independence and continued therapies:
* Promote self-care and self-management to achieve a maximum level of independence for service user. This may include: Provision of support to re-learn or find new ways of maintaining independence.
* Assistance with Service Users’ mobility, feeding, washing, bathing and dressing.
* Assistance with domestic care tasks such as laundry, ironing, and household cleaning.
* Preparation of food and drinks, kitchen cleaning and hygiene.
* Assisting Service Users to attend appointments, go shopping, collect pensions etc.
* Provision of emotional support, and support to increasing Service Users’ confidence and motivation.
* Assist with the use of existing equipment to promote independence, and provision of access to specialist equipment or technology as appropriate.
* Administer medication as designated in the care and support plan in accordance with Company policies and procedures.
* Assist Service Users with therapeutic activities as directed by the care and support plan and as directed by members of the multidisciplinary team.
Communication:
* Assist the healthcare coordinator and clinical lead nurse by communicating all information to ensure Service Users’ needs are met.
* Work in partnership with all members of Libertatem team and report any changes in a Service User’s mental or physical health, or social conditions.
* Ensure that concerns around safeguarding issues or equality and diversity are reported in line with Company procedures.
* Report any serious incidents, accidents or near misses to the office team, following company procedure.
* Report any hazards or potential hazards that impact on the safe delivery of care to the office team, in line with Company procedure.
* Report to the office team professional any concerns, complaints, or suggestions for improvements raised by Service Users and/or their Carers.
* Report to the office team promptly and appropriately to protect service users from neglect or abuse.
* Maintain clear, concise and legible Service User records, according to Company policy and procedure.
* Use Libertatem Healthcare’s ‘On Call’ system for additional support if required, in accordance with Company guidance.
* Ensure that all computer security procedures are followed.
* Always act in a manner which respects the confidentiality of Service Users, staff and any other involved parties.
Health and Safety:
* Every employee has a duty under the Health and Safety at Work Act 1974, to: Take reasonable care of the health and safety of themselves and all other persons who may be affected by their acts or omissions at work.
* Co-operate with Libertatem Healthcare Group to ensure compliance with Health and Safety legislation and the Health and Safety policies and procedures by not intentionally or recklessly interfere with, or misuse, anything provided in the interests of health, safety, or welfare, in pursuance of any of the relevant statutory provision.
Data Protection and Confidentiality:
The Support worker must at all times respect the confidentiality of information in line with the requirements of the Data Protection Act. This includes, if required to do so, obtain, process and/or use information in a fair and lawful way, and to disclose data or information only to authorised persons or organisations as instructed.
Training:
The Team leader will complete the mandatory and client specific training both in-house and external to achieve the specified level of competency for each client with whom they work. Prepare for, attend and participate in regular supervision and appraisal meetings with the healthcare coordinators and clinical lead nurses. Demonstrate a commitment to personal development. Team leader will be proactive in providing suggestions to clinical lead on any additional training that may benefit the team in delivering bespoke and client centered care and support.
