Job responsibilities
Your Key Responsibilities
Optimising Medicines & Supporting the Practice Team:
* Reduce GP workload: by taking the lead on medicines-related queries, prescribing reviews, and medication adjustments.
* Drive improvements in prescribing safety and quality: ensuring compliance with best practices and regulatory guidelines.
* Provide expert support to the wider practice team: offering guidance on complex prescribing decisions and medicines optimisation.
* Be a trusted point of contact for patients: ensuring they have access to safe, effective, and appropriate medication.
* Lead on antibiotic stewardship and medicines safety: supporting practice-wide initiatives to enhance patient care.
Clinical Duties:
* Medication Reviews: Conduct structured medication reviews, particularly for patients with polypharmacy, frailty, or multiple long-term conditions.
* Chronic Disease Management: Support patients with hypertension, Type 2 Diabetes, asthma, COPD, and other long-term conditions.
* Medicines Reconciliation: Ensure safe, effective transitions of care by reviewing medications when patients move between providers.
* Care Home & Frailty Reviews: Work proactively to reduce inappropriate prescribing and optimise medication regimens for vulnerable patients.
Dispensary Leadership & Development:
* Oversee and develop the practice dispensary: ensuring it operates efficiently, safely, and in line with regulations.
* Support and manage dispensary staff: providing mentorship, training, and ensuring compliance with audits and best practices.
* Drive quality improvement initiatives: identifying opportunities to enhance efficiency and patient experience in the dispensary.
Who we're looking for:
* A qualified Clinical Pharmacist and Independent Prescriber with experience in primary care or a keen interest in transitioning into the sector.
