Brokerage is a key part of the successful delivery of Adult Social Care. If successful you will support the delivery of excellent services to a wide range of internal and external stakeholders such as: People who use adult social care services, social workers, commissioners, providers of adult social care and health partners. The role will include: Supporting people with eligible adult social care needs to access the services they need by working with providers of adult social care. Working with colleagues to ensure peoples’ identified care needs are met in the most appropriate way. Supporting the brokerage service to contribute to ensuring the delivery of care is effective and safe. Making sure information is recorded accurately and in a timely manner to ensure quality management information and essential data is up to date. Building effective relationships with a variety of stakeholders Using a range of specific IT systems in order to carry out the functions of the role.
