Job summary HMT are looking for an experienced and passionate leader to drive the development and implementation of HMT's Reablement Strategy across our care homes and future community services. This role will play a key part in supporting our ambition to grow complex, niche services in community settings. As a clinical leader for Allied Health Professional (AHP) care, you will ensure care home teams deliver therapeutic interventions and activities to the highest standards. You will also support the implementation and monitoring of HMT's innovative Clinical and Care Practice Model & Framework, shaping the future of care excellence within our organization.
