Job responsibilities
19. Telephone Triage : Triages patients via online forms or telephone conversations and assesses the most appropriate care setting.
20. Assessment and Treatment : Assesses, diagnoses, plans, implements, and evaluates treatment for minor illnesses, undifferentiated diagnoses, or complex needs.
21. Emergency Care : Prioritises health problems and provides interventions in urgent or emergency situations.
22. Record Keeping : Maintains accurate and complete patient records in line with legislation and policies.
23. Quality Governance : Participates in quality governance and uses the audit cycle to implement improvements.
24. Nursing Services : Assists in organising and coordinating nursing services to provide evidence-based practice.
25. Acute Care : Works with the Duty Team to manage and care for patients with acute or urgent needs.
26. Long-Term Conditions : Identifies, diagnoses, and formulates treatment plans for patients at risk of long-term conditions.
27. Condition Management : Works with healthcare professionals to manage long-term conditions using evidence-based methods.
28. Medication and Follow-Up : Prescribes safe and effective medication, reviews adherence, and follows up based on healthcare needs.
29. Health Promotion : Supports patients in adopting health promotion strategies and self-care principles.
30. Vaccination : Implements and participates in vaccination programmes, including travel vaccinations.
31. Women's Health : Manages women's health needs, including family planning and cervical cytology.
32. Learning Environment : Promotes a learning environment, participates in training, and mentors learners.
33. Cover and Support : Provides cover for colleagues and offers support and mentoring for Health Care Assistants.
34. Meetings : Participates in or leads practice meetings and clinical team meetings.
35. Infection Control : Applies infection control measures according to guidelines.
36. Independent Prescribing : Independently prescribes to patients following protocols.
37. Diagnostic and Referral : Books diagnostic investigations, refers patients to other services, and interprets pathology reports.

39. Supervision and Training : Supervises or supports the training of junior clinical team members.

Qualifications
Essential
40. RGN Qualification: Must hold a valid RGN qualification
41. Independent Nurse Prescriber: Must be a qualified Independent Nurse Prescriber registered with the NMC
42. NMC Revalidation: Must meet NMC revalidation requirements as outlined in the NMC Revalidation booklet
Desirable
43. Qualifications:
44. For qualifications obtained after December 2020: A Master s degree is required with reference to RCN Credentialing for Advanced Level Nursing Practice
45. For qualifications obtained up to December 2020: A Postgraduate diploma or degree in Advanced Practice is required
