Your primary responsibilities will include prescription signing, medication reviews, reconciliation, and conducting safety searches to optimize prescribing practices and enhance patient care. You will work closely with GPs, nurses, and other healthcare professionals to support medication safety, chronic disease management, and deprescribing initiatives, ensuring compliance with clinical guidelines and improving patient outcomes.Key Responsibilities1. Prescription Management Sign and authorize repeat prescriptions in line with practice policies. Ensure prescriptions comply with best practice, local and national guidelines. Provide clinical oversight on high-risk medications and controlled drugs.2. Medication Reviews & Reconciliation Conduct structured medication reviews (SMRs), ensuring medicines are optimized for patient safety and effectiveness. Carry out medication reconciliation for patients following hospital discharge or care transitions. Address medication-related queries from patients and clinicians.3. Medication Safety & Searches Perform safety searches to identify and act on prescribing risks (e.g., drug interactions, contraindications). Implement medication changes in line with NICE, MHRA, and local prescribing guidance. Support audits and quality improvement initiatives related to medication safety.4. Clinical Collaboration & Patient Support Work alongside GPs and the healthcare team to manage long-term conditions such as diabetes, hypertension, and respiratory diseases. Educate patients on medication adherence and self-management strategies. Provide support for deprescribing where appropriate, reducing polypharmacy risks.5. Practice & Research Involvement Contribute to clinical governance, medicines optimization, and prescribing audits. Support research initiatives within the practice, improving evidence-based prescribing. Participate in staff training and mentoring junior pharmacists or trainees.Person SpecificationEssential Criteria: Registered Pharmacist with the General Pharmaceutical Council (GPhC). Independent Prescriber qualification. Experience in primary care, community pharmacy, or hospital settings. Strong knowledge of prescribing safety, clinical guidelines, and medication optimization. Ability to work independently while collaborating within a multidisciplinary team. Excellent communication and patient consultation skills.Desirable Criteria: Experience in structured medication reviews (SMRs) and long-term condition management.
