Main duties of the job This is an exciting opportunity to join the friendly GP practice team as an Advanced Clinical Practitioner for the Health Board Practice at CanolfanGoffa Ffestiniog, Blaenau Ffestiniog. We offer a Multi-disciplinary Team model, providing patients with the opportunity to see the most appropriate practitioner to effectively deal with their presenting condition at the first point of contact.The extended GP practice team consists of Salaried GPs, Practice Nurses, Health Care Assistants, Medicines Management Team, AdvancedPhysiotherapist and Occupational Therapist. The environment is of a happy and supportive nature with the team looking after each other and is alwayslooking at innovative ways of working. Clinical and peer support is available.We are looking for a person with a proven record/special interest in caring for patients with minor illness and some experience of dealing withChronic Disease Management.Ideally the candidate will hold an MSc in Advanced Clinical Practice or working towards this. This post will require the successfulapplicant to visit patients in their own homes, lead on Care Home visits, Palliative Care, CRTs and other general practice dependant on experience. The Health Centre is open between 8.00 and 6.30 pm, the post holder will be expected to work during these hours.
