An exciting and rewarding opportunity has arisen for a Fixed Term 1 year Band 6 position within the ALD South Tees Community team.
The post holder will work as part of the multi-disciplinary Community Learning Disability Teams, addressing and supporting the health, psychological, emotional, and behavioral needs of adults with a learning disability who have complex physical and emotional health needs. The base will be in Flatts Lane Centre covering Middlesbrough and Redcar and Cleveland area.
The focus of the team is to provide an alternative to in-patient care in line with the transforming care agenda, focusing on people with a learning disability who are experiencing (but not exclusively) mental health difficulties, behavioral challenges, placement breakdown, and medication titration who should be treated in the least restrictive environment with the minimum of disruption to their lives.
The team works collectively and collaboratively with others to ensure a high standard of care is provided and is always willing to go above and beyond what is expected when required.
We work predominantly 9-5 but there is an expectation to take on evening and weekend working plus on-call duties.
Main duties of the job
The successful candidate will have a genuine desire to improve the mental health and well-being of adults with learning disabilities and behaviors that challenge. They will work as part of a MDT delivering high-quality secondary care and enhancing community services, working with individuals who require specialist interventions using trauma-informed care & a recovery-based approach.
The successful candidate must be motivated, flexible, and able to communicate effectively. They will be expected to take part in the on-call rota and should be able to travel independently to various locations within the Trust on a regular basis.
Candidates must have 18 months post-qualifying experience including sign-off of appropriate preceptorship or competency framework.
The successful candidate will:
* Hold a complex caseload, completing assessments, delivering treatment and interventions, monitoring and evaluating outcomes, implementing Clinical Care Pathways.
* Provide clinical supervision, support, advice, and guidance to junior members of staff, students, colleagues, and others.
* Maintain good communication, documentation, providing reports and information within agreed timescales.
* Adhere to legal frameworks/policies and procedures e.g. Safeguarding, Mental Capacity Act, and Mental Health Act.
* Maintain their NMC Registration/PIN ensuring compliance with the NMC Code/revalidation.

Person Specification
Qualifications
* Current professional registration with Nursing & Midwifery Council - RNMH or RNLD (RNSC - CAMHS only) - RGN (If appropriate).
* Evidence of Continuing Professional Development.
* Recognized Practice Supervisor and/or Practice Assessor, clinical educator or equivalent. Must be achieved within 6 months.
* Trained in Clinical Supervision and providing clinical supervision OR willingness to work towards within 6 months.
Experience
* 18 months post-qualifying experience including sign-off of appropriate preceptorship or competency framework.
* Working collaboratively with service users and their families/carers.
* Providing clinical supervision to individuals or groups as professionally appropriate.
* Supervising or assessing students on practice placement.
* Working in a multi-disciplinary team.
* Quality improvement activities.
Knowledge
* Demonstrable knowledge to post-graduate level of evidence-based practice in caring for patients in the designated field.
* Understanding of relevant legislation (e.g. Mental Health Act, Mental Capacity Act).
* Detailed understanding of Safeguarding and its application in practice.
* Clinical Risk Assessment and Management and its application in practice.
* Clinical Governance and its application in practice.
* Research and development methodology.
* Understanding of psychological models of care and treatment.
* Working knowledge of clinical medicines management, including administration of injections.
