We have a fabulous opportunity for a full-time District Nurse (Band 7)/Senior Community Nurse (Band 6) and a full-time Senior Community Nurse (Band 6) to join Derby City Community Nursing teams. We have two posts within the Derby City North PCN and Derby City PCCO PCN.
Derby City covers a varied and vibrant demographic, making each day interesting. This is an ideal opportunity for a District Nurse who is ready for a change or an experienced registered nurse with varied experience who wants to develop their leadership and caseload management skills in a supportive environment.
Main Duties of the Job
The successful applicant will require community nursing experience, some knowledge of caseload management, and a high standard of community nursing knowledge and clinical skills.
You will be provided with training and support tailored to your needs to drive forward high-quality care and teamwork, including a thorough induction programme, regular one-to-ones, peer supervision, working within an MDT, and clinical training.
1. Provide effective evidenced-based care to a defined population/Primary Care Network through effective delegation, caseload management, monitoring of care standards, leadership, and performance management of team members.
2. Assess, plan, implement, and evaluate individualised patient care, utilising professional judgement at all times.
3. Provide clinical leadership and line management to less experienced nurses and other members of the multidisciplinary team in the delivery of high-quality care.
4. Contribute to the continuous improvement of community nursing within their Place, linked to the long-term NHS plan.

Person Specification
Education, Qualifications and Training
* Registered nurse with current NMC registration
* Relevant Nursing degree or equivalent relevant experience
* Have undertaken or be prepared to undertake a leadership development programme
Experience & Knowledge
* Caseload management
* Community experience
* Proven clinical expertise, knowledge, and experience in wound care, end-of-life care, and catheter care
* Knowledge of Public Health agenda
Skills and Attributes
* Commitment to the development of clinical practice of others
* Excellent communication skills both verbal and written
* Access to transport is essential for this post, as you will be expected to travel around various areas of Derbyshire to carry out required duties
* Able to work across 7 days
* Proven managerial skills and experience of change management
