An exciting opportunity has arisen for a self-motivated individualto join our Epilepsy Team. This role will be primarily based at our Great Horton surgery but willrequire travel to our other sites and various clinic locations across Bradford.

Main duties of the job
The primary focus of the role is to follow up with patients with a confirmed diagnosis of Epilepsy in line with NICE guidance, supporting appropriate management decisions and arranging further reviews or discharge.
The successful candidate will be required to work 37.5 hours per week.

Job responsibilities
1.Reviewfollow up patients in the community Epilepsy service.
2.Communicatecare needs with patient and the referring clinician.
3.Liaise withsecondary care teams as needed.
4.Participatein regular team meetings with the GPs with extended role in Epilepsy andsecondary care consultant.
5.Activelyparticipate in peer support network with other Epilepsy nurse specialists.
6.Deliver Epilepsyawareness and rescue medication training.
7.Sign postingpatients, families & clinicians where appropriate.
8.Provide written management protocols forpatients and their carers.
9.Offer high quality evidence based care to patients with Epilepsy.
10.Enable MDT approach to care of patients with Epilepsy.
11.Facilitate an efficient & accessible service for Epilepsypatients.
