A position has arisen for an enthusiastic and motivated individual to undertake the role of a part time Practice Nurse to work alongside our current practice nurse. We are ideally looking for between 20 to 25 hours. The ideal candidate should be registered with the NMC, must be working or have had practice nurse experience, and provide certification of training courses undertaken for the relevant post.
Main duties of the job
To offer a high standard of evidence-based care to patients, to include health promotion, treatment, screening, and disease management. Specific clinical areas include:
1. National immunisation and vaccination schedule
2. Travel Immunisations and Vaccinations
3. Essential hypertension/CKD/CHD
4. Cervical smears
5. Urinalysis
6. Chronic Disease Management
7. Home Visits

We are an established, friendly and supportive GP Training Practice based in Rainhill, Merseyside. We have a patient list size of around 9000, with a team of 3 GP partners, 2 salaried GPs, a practice nurse, and a Healthcare Assistant, along with an experienced Administration and Reception Team who offer a wide range of support. We use EMIS web clinical system along with Docman 10 and other clinical systems. We pride ourselves on delivering excellent care to our population.
Job responsibilities
This is not a comprehensive list but outlines minimum requirements:
1. Provide general nursing care to practice population who are referred or self-refer in the practice or in home setting including assessment, planning, delivery and evaluation of:
* Administer nebuliser
* Spirometry/respiratory testing
* Cervical cytology
* Blood pressure and other physiological measurements
1. Help the practice develop and deliver specialist primary nurse-led services including chronic disease management.
2. Initiate the role of diagnosis for patients suspected to have a chronic disease e.g. diabetes, COPD, asthma, and CHD, referring to other clinicians as appropriate.
3. Manage programs of care for patients with chronic diseases by planning, providing, and evaluating care under agreed guidelines and patient group directions.
4. Maintain disease registers in liaison with administrative staff.
5. Undertake annual review of patients' understanding and ability to self-manage.
6. Provide comprehensive travel health advice for patients and administering of travel vaccines.
7. Administer child and adult immunisations and vaccinations in accordance with national and local programs.
8. Competent in anaphylaxis and resuscitation techniques.
9. Ensure safe storage, rotation, and disposals of vaccines and drugs and that adequate stock levels are maintained.
10. Undertake autonomous comprehensive assessment of patient need and develop care plan in accordance with local and national protocols.
11. Make judgements on a range of complex issues requiring analysis, assessment, and solution.
12. Deliver care to specialist groups in accordance with practice protocols and QOF guidance, local and national guidance.
13. Provide health screening to practice population.
14. Autonomously refer to multidisciplinary team for referral/treatment/discharge.
15. Visit patients in their homes to deliver primary nurse-led services.
16. Safely use and manipulate equipment used in examination and treatments.
17. Computer literate with practice patient record system and electronic communication systems.
18. Safely manage hazardous biological substances including blood/urine/vomit/faeces/vaginal secretions.
19. Communicate effectively with patients, relatives, and carers regarding treatment and progress of patients, maintaining confidentiality of all patient information.
20. Manage issues arising from sensitive situations e.g. unwanted pregnancy, sexually transmitted infection, child protection issues, mental health.
21. Act as patient advocate/chaperone.
22. Aware of and provision of evidence-based care.
23. Physically handle patients to assess/treat in emergency situations.
24. Provide home visits to patients.
25. Health and safety.
26. Infection Control lead.
