An exciting opportunity has arisen for a Clinical Nurse Specialist to join the Urology team in a Band 6 role.
This post will report to the Urology Outpatients Manager and work closely alongside the Consultant and ANP team. The successful applicant will join a well-established clinical team with the opportunity to engage in further training and development.
Assessing patients with complex urological disorders. Organising plan of care to including removal of catheters and long-term catheterisation.
Liaising with District nursing/Community Nursing Teams.
Liaising with MDT re: patients plan of care.
Mid Cheshire Hospitals NHS Foundation Trust (MCHFT) provides good quality, safe and effective healthcare to the people of Cheshire and beyond. The Trust, which manages Leighton Hospital in Crewe, Victoria Infirmary in Northwich, and Elmhurst Intermediate Care Centre in Winsford, was established as an NHS Trust in April 1991 and became a Foundation Trust in April 2008.

CLINICAL SKILLS
1. Lead a specialist area of practice, providing expert professional advice to others across the Trust within agreed guidelines and protocols, including medical colleagues and other professional groups when required.
2. Perform comprehensive specialist assessment of patients, plan, treat and evaluate care within agreed guidelines and protocols according to evidence-based practice and changing health needs.
3. Communicate complex specialist information to medical colleagues and others. Refer patients to appropriate clinicians, in the Trust or externally, within agreed guidelines and protocols, according to assessment of patient need and evidence-based decisions.
4. Prioritise patient care, supervising and allocating one’s own workload, monitoring and evaluating effectiveness of care, including discharge of patients, or referring on, within agreed guidelines and protocols.
5. Perform specialist clinical skills in the treatment and overall management of patient care, including requesting and interpreting investigations, within agreed guidelines and protocols.
6. Provide a comprehensive specialist service, accepting accountability and responsibility for the management and effectiveness of the service on a day-to-day basis.
7. Collect, collate, evaluate and report on information, including communicating detailed information to clinical colleagues and others, maintaining accurate patient records and documentation.
8. Promote patient and public involvement in the improvement of services and involve patients and carers/relative in the planning and delivery of their care.
9. Work collaboratively with other professionals and agencies to ensure patient needs are met, especially in relation to patient flow, care pathways and discharge arrangements.
10. Analyse and act on information to proactively manage and maintain NHS targets and standards.
GOVERNANCE
11. Establish and maintain effective communication, with patients and their families maintaining confidentiality of information, dealing with difficult people and situations as they arise, including providing emotional support to patients, families, and junior colleagues in dealing with frequently distressing circumstances including sudden and unexpected death and the breaking of bad news.
12. Monitor and report on quality standards within own specialist area of expertise, e.g., identifying risks, bench-marking, audit, preparing and implementing of action plans to improve quality of services.
13. Work with Divisions/Director of Nursing and Patient Care Standards to ensure that lessons are learnt from issues arising from risk reports/complaints etc, and that they are promptly and positively addressed.
14. Lead on the development and implementation of policies, procedures and guidelines related to own area of specialist practice and contribute the development of other policies.
15. Ensure compliance with Trust policies, procedures and guidelines for self and others, by taking action/alerting senior management team if practice appears to contravene policy, or if concerned about any aspect of patient care.
MANAGERIAL/LEADERSHIP
16. Act as a role model by demonstrating leadership and expertise, and by maintaining credibility across the Trust, the wider health care community, and external agencies, ensuring a positive image of the Trust is maintained.
17. Lead and/or participate in meetings internally or externally to the Trust, relevant to specialist area of practice, proactively contributing to the meeting by providing advice and expertise in own specialist area.
18. Work with Divisions/Director of Nursing and Patient Care Standards in providing managerial support, participating in the recruitment, induction, and orientation of new staff, carrying out appraisal and clinical supervision, and by ensuring that mandatory training take place according to Trust policy.
19. Ensure effective use of resources, e.g., staff, budget, premises, equipment, supplies and materials.
20. Maintain systems and processes to promote a healthy, safe and secure working environment, ensuring compliance with legal and regulatory requirements, maintaining accurate documentation and reporting any concerns.
EDUCATION/LEARNING
21. Take responsibility for own learning and development by recognising and taking advantage of all opportunities to learn, including full participation in KSF/appraisal, supervision, action learning and by maintaining a professional/personal portfolio of learning.
22. Act as a specialist resource, providing support, guidance and knowledge across the Trust, utilising evidence-based theory and practical experience gained.
23. Provide advice and specialist training from a sound knowledge base, to a range of staff, including junior medical staff, and evaluating the impact of learning on changing practice.
24. Initiate and conduct audit activity by continually monitoring standards of care, identifying risks, advancing knowledge within own specialist field, bench-marking, assisting with clinical trials, gathering, collating and interpreting data and information, using a wide range of research methodology.
