An exciting opportunity has arisen for a permanent Band 7 Specialist Palliative Care Nurse, 30 hpw (0.8WTE) to join the Palliative Care Hospital Liaison Team as part of the Palliative Care Service in Northumbria Healthcare Trust. The post will be Trust wide covering Northumbria Specialist Emergency Care Hospital (NSECH) in Cramlington and Wansbeck General Hospital, Ashington and may at times have to cover into another area of Northumbria. The Team support medical and nursing staff in caring for palliative patients with complex needs in an acute hospital setting. The service will cover 7 days a week extended working hours service (8am until 8pm) and the post holder may be part of the staff rota to cover these hours which includes weekend and Bank Holiday covering only at NSECH during the extended hours.

Main duties of the job
1. Provide expert specialist palliative care for patients and carers with life-limiting illnesses within the hospital settings.
2. Develop partnerships with members of the Multidisciplinary Team in the Trust to provide a seamless service of palliative care, enabling patients to be maintained in an environment of their choice.
3. Provide clinical professional support, resources, and education when necessary to members of the Multidisciplinary Team.
4. Work as an autonomous practitioner within a multidisciplinary environment, ensuring adherence to professional codes of conduct.
5. Work with other professionals to promote and develop the ethos of Palliative Care within the Trust and the local community.
