We are looking to recruit an experienced Practice Nurse to enhance our current nursing team, here at Ashworth Street Surgery.
Ashworth Street Surgery has a registered population of over 16,000 patients.
We are a large team of 8 partners, Salaried GP's, ANP, Community ANP, Practice Nurses, Health Care Assistants and Phlebotomists.
Main duties of the job
You will have experience in Chronic disease management including diabetes, Childhood Immunisations, Vaccinations including Travel, Cervical Cytology, Contraception management, Cancer Care reviews, Spirometry and Feno.
The successful applicant will work alongside our other 4 Practice Nurses and provide support to junior members of the nursing team including HCA's and Phlebotomists to ensure high quality care.
You will work to Infection prevention and control standards and cold chain compliancy, attend and contribute to internal nurse meetings and represent the Practice at PCN/locality Practice Nurse meetings and training.

Ashworth Street Surgery was formed in July 1999. There are eight doctors who form the partnership: Dr's Steve Doyle, Dave Osborne, Shalini Gadiyar, Mark Mudalige, Adam Simmons, Kevin Cody, Hassan Riaz and Jamie Oldfield.
There are also a large number of receptionists and administration staff who play a very important role in the everyday running of the practice.
We are a large training Practice with 3 GP trainers. Training and supporting our attached learners is one of our core principles.

Clinical Practice
1. Assess, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being.
2. Implement and evaluate individual treatment plans for patients with a known long-term condition.
3. Identify and manage as appropriate, treatment plans for patients at risk of developing a long-term condition and support health prevention programmes.
4. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
5. Support patients to adopt health promotion strategies that encourage patients to live healthily, and apply principles of self-care.
6. Deliver opportunistic health promotion using opportunities such as new-patient, over-40s and well-person medicals.
7. Provide sensible and up-to-date information and advice on prescribed or over-the-counter medication on medication regimens, side effects and interactions, seeking clinical support as appropriate.
8. Support and advise women requesting information relating to family planning needs.
9. Support and manage health needs of women presenting for cervical cytology consultations.
10. Recognise, assess and refer patients presenting with mental health needs in accordance with the National Framework (NSF) for Mental Health and the Mental Capacity Act (MCA).
11. Implement and participate in vaccination and immunisation programmes for both adults and children.
12. Advise, support and, where appropriate, administer vaccinations for patients travelling abroad.
13. Promote and deliver evidence-based care for patients presenting with aural conditions.
14. Assist GP colleagues in providing minor-surgery sessions where required.

Qualifications
* Good standard of general education.
* Registered Nurse.
* Membership of professional body.
* Diabetic Diploma.
Experience
* Experience of working within a nursing team.
* Experience of dealing with vulnerable patients.
* Experience of using own initiative.
* Practical experience of computerised recording system.
* Experience of working within General Practice.
