An opportunity has arisen for a driven and innovative Assessment and Review Worker to join People’s Services within Contact Cares. This large and busy multi-disciplinary team and is the single point of access for health and social care referrals. As the team continues to develop, we are looking to recruit a motivated Assessment and Review worker to join our assessment team within the service. The team consists of both qualified and unqualified Social Care, Nursing and Therapy staff. We offer planned and urgent assessments and provide a 2-hour response under Urgent Community Response Criteria.
The Contact Cares team also provide initial contact, care planning, onward referrals, and signposting services for a wide range of adult clients across health and social care. With a focus that stretches from prevention and early intervention, to reducing hospital admissions and readmissions the work is varied and interesting.
The team operate from 8.00 am until 10.00 pm 7 days per week to include Bank Holidays. This post will be shift working to a rota over 7 days.
