Our care coordinator will: Working with the primary care team and using the clinical IT system to identify cohorts of patients who require annual health reviews. Contact and work with these patients to organise the review appointments. Help plan, administrate appointment rotas and help with the administration of these roles and clinics. Contribute to improving healthcare outcomes by working with our Quality leads on Quality Outcomes Framework (QOF) within the practice. Assist patients to navigate and progress through the relevant pathways, processes and systems; answering patient questions, making phone-calls, arrangments for care, liaising with professionals, service personnel and wider stakeholders. Implement processes for safety-netting at the practice so that patients are followed up effectively. Contribute to the implementation and streamlining of new pathways for effective follow up of patients with long term conditions. Work with other PCN care coordinators to share learning and best practice. Support Carer's Leads across the PCN in developing groups, support and resources. Build and maintain effectively relationships with practice teams and PCN Staff. Collate patient data and update patient records. Create, amend and use searches within the clinical system to identify cohorts of patients. Produce a variety of information for audit in various formats such as spreadsheets. Participate in Practice meetings. When needed provide support to the administration team by way of document processing, coding records and various other admin functions. Additional work will be delegated by the Practice Manager. Ensure awareness of responsibilities in relation to health and safety, infection control and all governance/safety policies. To be willing to work additional, extended and improving access hours when requested/required. Any other duty as shall be reasonable be required. The Ideal Candidate You should be highly efficient administratively. You should have excellent interpersonal communication skills and be able to deal with patients sensitively. You should be able to speak professionally and confidently to different types of patients, colleagues and clinicians. You should have an understanding of primary care or have been in an NHS setting in the past. You should be proactive and able to work independently, using your own initiative, but able to take and respond to detailed instruction where required. You should be able to prioritise multiple tasks and achieve them all to a high standard at all times. You need to have a 'can-do' attitude and be willing to take on new challenges positively.
