We are recruiting for a Staff Nurse - Discharge Lounge who shares our ambition and vision to deliver the Best of Care by the Best People and has the drive to take us forward, ensuring that the Trust continues to be the first choice for patients and our staff.
Main duties of the job
The Discharge Lounge is a dynamic inpatient area which accepts patients ready to leave the hospital, waiting for medications and/or transport home. As a Registered Nurse working in the Lounge, you will be expected to:
* Take initiative to proactively seek patients suitable to come to the lounge using the TeleTracking system.
* Contact wards to obtain handovers.
* Communicate effectively and professionally with colleagues and services.
* Organize patients' transfers to the Lounge.
* Challenge any gaps in the plans for discharge.
* Complete medication rounds for patients in the Lounge when due.
* Keep patients informed on the progress of their discharge.
* Liaise with the pharmacy department and teams to follow up on the progress and dispensing of patients' medications.
* Check patients' TTOs and assure they are complete.
* Liaise with the hospital Social Services/Discharge team, care homes, and other hospitals to assure that patients are discharged to the right places and by the right time.
* Communicate with NOKs to assure they are kept up to date with discharge plans and the time of patients' leaving the hospital.
* Document the care provided for patients and complete the Discharge Checklist for every patient leaving the hospital.
* Ensure patients receive good care and experience while in the Lounge and complete Friends and Family surveys with patients to support that.
* Promote teamwork and maintain good relationships with colleagues.
* Use the transport booking electronic system to book and track patients' transport.
* Recognize signs of deterioration in patients, complete their observations, and escalate appropriately.
* Liaise closely with Site practitioners to escalate any issues surrounding patients' discharge and maintain safe and effective hospital flow.
The Discharge Lounge will provide you with the opportunity to expand your knowledge and skills in the discharge process and practices.
Person specification
Qualifications
* RN1/RN2/RN12 or RN Child Registered Nurse on the NMC register.
* Evidence of continuing professional development.
Experience
* Experience of working independently and in a team.
* Experience of working in the relevant area.
* Supervision of junior staff.
Knowledge
* Up-to-date knowledge of current clinical and professional issues.
* Ability to use own initiative and make decisions.
* Knowledge of different discharge pathways.
* Understanding of Audit and Research Based Nursing Practice.
* Knowledge of the NMC Code of professional standards of practice and behaviour for nurses and midwives.
* Knowledge of Clinical Governance.
* Awareness of individual responsibility towards health and safety.
Skills
* Able to assess, plan, implement, and evaluate programs of care.
* Organized and able to prioritize accordingly.
* Ability to administer prescribed medication including transfusion of blood and blood products.
* Basic computer skills including ability to use Microsoft Word and awareness of using the TeleTracking system.
* Able to recognize and escalate deteriorating patients.
* Able to work as an effective team member.
* Able to supervise and teach junior staff.
* Able to manage difficult situations.
* Excellent verbal, written, and interpersonal communication skills.
