An exciting opportunity has arisen for an experienced Advanced Nurse Practitioner to join our unique, highly skilled VAWAS (Vulnerable Adult Wrap Around Service) team. The role of the VAWAS ANP requires an experienced nurse who, acting within her/his own professional boundary, will provide care for patients within all the Nursing and Care homes in the localities, and all other patients, within their own homes who have been referred into the scheme by GP practices within the above boundaries. The ANP will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures.
Main duties of the job
Assess patients presenting with acute illness living within the Nursing and Care Homes within the locality who are at risk of inappropriate hospital admission. Diagnose, plan, implement and evaluate treatment, interventions and care for all patients presenting with an undifferentiated diagnosis and those with complex needs. Clinically examine and assess patient needs from a physiological, psychological and sociological perspective, and plan clinical care accordingly. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate). Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan. Refer patients to other medical specialities for assessment if deemed appropriate. Undertake and interpret a range of diagnostic tests and routine clinical procedures. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols and formularies. Work with patients and their carers in order to support compliance with and adherence to prescribed treatments.

Jubilee Medical is an established GP practice based in Newton Aycliffe with a branch site in Shildon. The practice is passionate about providing additional services to our local community, currently offering consultant-led Paediatric and ENT clinics, out of hours access and home visits (including weekends and bank holidays) to all patients registered within our Primary Care Network. We also have a dedicated team of Advanced Nurse Practitioners supporting our local care homes ensuring our elderly population receive the appropriate clinical care and monitoring on a weekly basis. The practice has been operating as a Covid Vaccination Site since December 2020.
Job responsibilities
The role of ANP requires an experienced nurse who, acting within her/his own professional boundary, will provide care for patients within all the Nursing and Care homes in the localities, and all other patients, within their own homes who have been referred into the scheme by GP practices within the above boundaries. Acting as an autonomous practitioner working independently, and in conjunction with, other health care professionals to assess, investigate, diagnose, treat and evaluate all patients seen, including referring into other services wherever or whenever required. The post holder will demonstrate safe, clinical decision-making and will provide expert professional advice to patients, carers and colleagues ensuring maintenance of clinical excellence. The ANP will work collaboratively with the general practice team to meet the needs of patients. They will provide education and training to other staff and students, and provide leadership as required.
