NHS Scotland is reducing their full time working week from 37.5 to 37 hours per week from 1 April 2024 but with no change in pay. This reduction will also be applied pro rata for part time staff. Salary Scale: £46,244- £53,789 pro rata
West Lothian Health and Social Care Partnership are looking for a dynamic, highly motivated and enthusiastic pharmacist to support delivery of pharmaceutical care in both primary and secondary care, contributing to:
1. Working with hospital at home and other teams in West Lothian where appropriate
2. Provision and support of pharmaceutical care initiatives in primary care in co-operation with primary care, social care and community pharmacies e.g. medication reviews, improving compliance and polypharmacy reviews as part of wider multi-disciplinary team.
3. Liaising with secondary care acute and St Johns Hospital.
4. Providing direct pharmaceutical care to older people with complex needs both in primary care and in community hospitals.

The NHS Lothian Primary Care Pharmacy and Medicines Service is undergoing an ambitious programme of transformation and integration. Pharmaceutical care is provided by a team of around 45 pharmacy staff, including pharmacists, pharmacy technicians and administrators, working across General Practices and Pharmacotherapy Hubs covering the whole of West Lothian.

Who you are:
You are a dynamic, highly motivated and enthusiastic, GPhC registered pharmacist.
You may have a community or hospital pharmacy background, be working within a GP Practice as a pharmacist or already be working with a hospital at home team. Candidates will have gained postgraduate experience working in a patient focused clinical environment, using their knowledge and skills to improve patient care and promote optimal outcomes from medicines.
You should have excellent interpersonal and communication skills and demonstrate leadership and innovation consistent with NHS Lothian's values of quality, teamwork, care and compassion, dignity and respect, and openness, honesty and responsibility. You will have experience of involvement in the development and education of others.
The ideal candidate will be an experienced pharmacist independent prescriber and will be able to work autonomously as well as part of a team.

This post will give you the opportunity to continue to develop your clinical skills as part of the Hospital at Home and Community Hospitals team covering West Lothian, to provide holistic, patient-centred individualised care to improve the safety and effectiveness of individuals medicines.
If not already completed, there will be opportunities and support for successful applicants to complete the General Practice Clinical Pharmacist Competency & Capability Framework, a post graduate clinical diploma and the independent prescribing qualification. You will receive support from senior pharmacists, as well as peer support from the wider pharmacy team. You will have ringfenced development time during your working week.
