We are looking for an RMN with a desire to develop their skills with Older Adults. The ideal candidate will be motivated and have a genuine desire to extend their skills and expertise in holistic aspects of assessment and treatment for dementia and functional mental illness.
You will be working with an established multi-disciplinary team in busy clinics and patients' homes; therefore, a car driver is essential for this post.
You will need to be able to demonstrate excellent organisational and time management skills alongside clear communication and a desire to learn and develop the services we provide.
We will provide robust clinical and managerial supervision in order to support the successful candidate in their development as a Band 5, and as part of the Trust's preceptorship programme.
This integrated CMHT provides a Memory Assessment Service, care for patients with more complex needs associated with both functional and organic illness, and Care Home and Hospital Liaison. The unique set-up of these integrated teams allows a seamless delivery of care for patients throughout all stages of their illness, and a collaborative approach within the team.
The team serves the population of the Furness Peninsula at the edge of the Lake District and has its base in Barrow-in-Furness. It is an established team with a friendly and supportive culture. There is a visible and supportive leadership team who are committed to clinical supervision and professional development.
Main duties of the job
* Provides assessment, advice and treatment to people with Dementia and complex mental health needs referred to Community Services in accordance with care pathways for given specialism.
* Delivers interventions in accordance with care pathways for given specialism.
* Works closely with other professionals within and outside the Community Team.
* Actively supports the Team Leader in the achievement of KPI / CQUIN and related County Council targets, and maintenance of registration with regulating bodies.
* Takes the lead on specific objectives as directed by the Team Leader.
* Provides supervision and direction to support staff within community services.
* Contributes to the screening of referrals and allocation to service lines.
* Supports a culture of ongoing learning and development on an individual and team basis to ensure high standards are maintained and, where appropriate, contribute to the delivery of training as appropriate.
* Provides leadership to junior colleagues acting as a positive role model.
