Calling band 6 Community Mental Health Nurses!

We are looking for passionate nurses who will focus on providing support and high-quality nursing care for clients and carers, targeting and prioritising people with severe mental health problems to join the NW Community Mental Health Team for Older People (CMHT OP).
Based at Unither House in Chertsey, the CMHT OP service offers assessment, diagnosis, treatment and support for people over 65 who present with functional mental health illnesses deterioration in cognition and/or dementia. We work closely with other services, such as social care and also with the voluntary sector, particularly the Alzheimer's Society, to provide positive support for people using services and their carers, ensuring effective forward signposting where appropriate.
Main duties of the job
* To act as key worker / care co-ordinator for named individuals, within the Care Programme Approach process, including Single Assessment
* The ability to work alone
* To provide high standards of care, which are delivered in a professional manner.
* To ensure that the care delivered is based on best practice, and is regularly reviewed.
* To ensure all records and reports are kept up-to-date, and up to the standard expected of this grade of nurse, in accordance with the Caldicott Report.
* Keep an accurate, up-to-date diary and record of visits.
* Be responsible for the continuous overall management of a defined caseload.
* To receive and give clinical and management supervision / guidance
* To supervise juniors / other staff in the delivery of care and treatment to individuals and groups
* To support junior staff to manage caseloads effectively
* To act as a positive role model for junior staff, assisting / guiding them in their duties
* To provide information to the Line Manager, if any incidents that may affect the standing or running of the service
* To develop close working relationships with voluntary / statutory services, in particular Social Services and members of the Primary Care Team, and participate in the system of integrated team working
* To educate and train junior staff / student nurses and other professionals around competencies of a Community Mental Health Nurse
* To participate in the Duty Rota to manage new referrals

* RMN/BSC Hons or Diploma in mental health nursing 2 years post qualification experience Membership of a professional body (NMC registration)
* DRIVING: Must be able to drive for work purposes

* Further training or experience to post graduate diploma level or 5 years equivalent experience
* ENB 811, 812 or equivalent Community Nursing course
