Job Title: Social Worker - Front Door team - Stockport Location: Stockport Pay Rate: £30 Liquid Personnel is currently recruiting for a Social Worker to join Front Door Team based in Stockport. What will your responsibilities be? In this role, you will Work with Health colleagues to reduce hospital attendance, complete visits (including same-day response), conduct Care Act and short-term assessments to promote independence, provide Social Work expertise in a multi-disciplinary team, and maintain office-based role requiring a clean driving license and transport.
