Carer Support Coordinator Job Vacancy (Darlington)
We have an exciting opportunity for a Carer Support Coordinator to join our Darlington team.
17.5 hours per week, one year fixed term contract with possible extension. Actual Salary £12,462.05
Covering the Darlington area (hybrid working available)
We are seeking a highly motivated individual with a passion for helping others and a solid understanding of the challenges faced by Carers. The successful candidate will deliver information and support to Carers through various channels, including telephone, online, and face-to-face appointments in their homes and community venues. You will provide support by working in a solution-focused manner that aims to empower Carers to make informed choices while enhancing their health and well-being.
Main Tasks and Responsibilities:
* Information and Support: Deliver information, support, and advice to Carers via telephone, in-person meetings, email, written documents, and virtual platforms such as Zoom, Teams, and Skype.
* One-on-One Support: Provide individualized support to Carers as needed and conduct support assessments for Carers accessing DCCS services using the Carers outcomes tool.
* NHS Carer Break Funding: Ensure Carers are informed about NHS Carer Break funding, its criteria, and assist in completing relevant application forms for those eligible.
* Caseload Management: Manage your own caseload, maintaining up-to-date records and monitoring as required using Charitylog.
* Referrals and Signposting: Refer Carers to Social Care Direct for comprehensive Carer assessments when appropriate and direct Carers to relevant organizations that can offer additional assistance based on their individual needs.
* Identifying Hidden Carers: Connect with GP surgeries, social care services, libraries, and community services within the allocated area to identify hidden Carers and provide them with information about DCCS services.
* Welfare Benefits Support: Help Carers access appropriate welfare benefits to maximize their income and support their caring roles, including assistance with form filling.
* Grants and Bursaries: Support Carers in financial hardship with grants and bursaries for essential household items.
* Stay Informed: Keep up to date with changes in welfare benefits, Carer policies, and service provisions.
* Collaboration: Work collaboratively with fellow Carer Support Coordinators to share knowledge and skills.
* Meetings and Training: Attend meetings, training sessions, and events as required.
